About this service
Powers of attorney, healthcare directives, and estate documents signed at a hospital bedside, a care facility, or at home on hospice. We are comfortable in these settings and we do not rush. The travel fee is based on distance from Brimfield, from $35 to $95, plus $5 per notarial act. After you book, we’ll send a link to pay the travel fee, and your appointment is confirmed once it’s paid. Notarial fees are paid at the visit by card, cash, or check. Call (309) 431-1850 if you need someone today. The signer must be able to understand what they are signing and to sign willingly. Every signer needs valid, unexpired photo ID. If ID is a problem, call us before you book and we will talk through the options. If the signer can use a phone or tablet, you can also meet online by secure video call.
Cancellation Policy
I know these visits often come together quickly and plans can change. If anything changes, call or text (309) 431-1850 as soon as you can. Your appointment is confirmed once the travel fee is paid. If you cancel or reschedule at least 24 hours before your appointment, the travel fee will be refunded in full or moved to your new time. Within 24 hours, the travel fee isn't refundable, and a new time needs its own travel fee. Appointments booked within 24 hours are final once the travel fee is paid. The travel fee also isn't refundable if the notarization can't go forward once I arrive, for example because of an expired ID or a missing signer. Full details: kitchentablenotary.com/booking-policy
